New features for the intraoperative OCT system

A number of recent innovations have made handling easier and simplified the surgical workflow

“Those who stop to get better stop being good!” Philip Rosenthal was already aware of that in 1916, and engineers at HAAG-STREIT SURGICAL internalise this maxim. Physicians all over the world got to test the intraoperative OCT system for its functionality over the past months. Picture-in-Picture–Superimposition (C.INJECT)

The intraoperative optical coherence tomography (or iOCT™) allows the surgeon to directly assess structures within the tissue while performing the surgical operation. As of late, these iOCT images can be displayed synchronically using the microscope overview, which means that the surgeon gets to see a picture-in-picture superimposition. This synchronicity aids the physician in navigating the area under treatment and makes the operative workflow enormously easier. In addition, the iOCT is linked to the zoom and the lens focus of the surgical microscope, which offers the same field of vision by way of the microscope as well as with iOCT.

The preliminary experiences with the C.INJECT show that the superimposition on both eyes is best adjusted to the special needs of the surgeon. Physicians were particularly impressed by the high resolution of the iOCT- scans.

Simultaneous video recording of image data secured from the camera and iOCT

Using M.REC2, it is now possible to make parallel video and photo recordings from the camera and iOCT-image. This allows the surgeon to have one image in the full screen mode, while the other is displayed at reduced scale. The scale and position of the small window can be individually adjusted. It goes without saying that the channels can also be adjusted. Using the recording and snapshot function, the system saves each camera and iOCT recording separately as full HD videos or single high resolution images. Using M.AED, a processing program, a shorter version of the recordings that in part extend to hours can be made to highlight the most significant surgical steps.

Christopher D. Riemann, M.D., from Cincinnati Eye Institute in Ohio confirms: „The ability to simultaneously record time synched surgical video and iOCT video is a very welcome addition to the HS MIOS 5 microscope operating system and M.REC 2 recording system. Workflow enhancements for video acquisition are sorely needed in ophthalmology and the HAAG-STREIT SURGICAL offering in this space is just superb.”

New foot switch function

Many users now take advantage of the foot switch to operate the microscope during the operation. They appreciate the option of customizing the key functions according to their own needs with functions such as zoom, snapshot or video recording. As of late, it is also possible to regulate the iOCT functionalities and the picture-in-picture superimposition that way.

Setting individual user profiles

As of now, the described settings can be saved separately for each physician in user profiles. During the initial settings, the individual start-up preferences or, by way of example, the selection of the optics, can be retrieved and if necessary adjusted intra-operatively.

New user interface of the iOCT software

In order to design user-friendly menu navigation, the push buttons were labelled more clearly and the colour coding was adjusted to the design of HS MIOS 5.

NEW: iOCT atlas and iOCT booklet

For interested physicians, HAAG-STREIT SURGICAL has summarized all case studies and user assessments in a brochure (iOCT Booklet) and many iOCT videos, 3D scans and picture-in-picture superimpositions, which can be accessed from the website (iOCT-Atlas: www.iOCT-Atlas.com). The iOCT booklet was published for the first time on AAO in Chicago on 18 October 2014 and can be ordered at no charge as PDF or hardcopy.

Intraoperative optical coherence tomography (iOCT™) allows the surgeon to secure a non-invasive, contact-free imaging to display structures and layers within the tissue. The iOCT camera is remarkable owing to its premium iOCT scans shown with grey scale values in real time.
