Revolutionary Shoulder Surgery with mediCAD

Leonberg Hospital performed its first shoulder surgery with the assistance of the HoloLens.

In early July, Leonberg Hospital successfully conducted its first shoulder surgery with the assistance of mediCAD 3D software, benefiting from an innovative intraoperative representation of the surgical plan.

Dr. Rieske, Head of the Department of Orthopedics and Trauma Surgery, collaborated with our experts in planning the procedure using mediCAD 3D Shoulder Software. Special emphasis was placed on the optimal placement of implants and the resection line for shoulder arthroplasty in the presence of an existing humeral head fracture.

After thorough analysis, the team opted for a reverse shoulder prosthesis. The automatically generated 3D model quickly determined the ideal positioning of the implants. By simulating the patient’s range of motion, potential bony interferences in the planned movement radius could be ruled out.

The surgical plan was exported both as a 3D model and as a detailed planning report with dimensions, and transmitted to the two HoloLenses used at the start of the procedure. During the surgery, both surgeons had access to the same hologram of the planned outcome, utilizing the 3D model to observe and understand the position of the humeral head fragments.

A key question was the correct placement of the K-wire and baseplate on the glenoid to achieve the planned implant position. Thanks to the 3D model of the scapula, the surgeons could understand the glenoid’s structure despite soft tissues obscuring the patient’s anatomy. The Friedmann line displayed on the hologram served as an additional reference point to achieve an optimal outcome.

The real-time transmission of the surgeon’s perspective via video enabled the rest of the surgical team to follow the current status of the procedure and prepare for the next steps promptly. Ultimately, the surgery was successfully completed, thanks to the intraoperative support of the latest mediCAD® software. The users were thrilled with its user-friendliness and intuitive operation.
