Last year, the "consensus of Emergency Laboratory Capacity Building and Standardization of Chinese Experts" was jointly issued by the Laboratory Medicine Branch of the Chinese Medical Association, the Emergency Physician Branch of the Chinese Medical Doctor Association, and the Emergency Medicine Professional Committee of the People's Liberation Army.

Experts put forward specific and feasible requirements for the construction of emergency examination, department management and quality control in the “consensus”. In China, most patients with acute onset, severe symptoms, and complex and diverse conditions are often diagnosed in the emergency department firstly, making the emergency department a department where acute and critical diseases gather.

So what requirements does the “consensus” put forward for the detection of emergency coagulation?

Requirement 1:

“The detection system has a strong anti-interference ability to common influencing factors such as hemolysis, icterus and lipemia..."

“TAT in the emergency laboratory meets at least: blood, urine, stool routine items ≤ 30mins..."

The key to choosing the appropriate method to the "breaking point" of coagulation detection: the magnetic bead method, which effectively avoids the interference of hemolysis, but the cost of use is high; although the optical method is susceptible to interference from the samples, it has a lower cost and higher sensitivity, and is currently widely used.

Requirement 2:

“Regular operator comparisons are carried out according to the requirements, and the same test items between different instruments are compared to ensure the quality of emergency examination..."

Coagulation testing is primarily for the overall interpretation of the coagulation process. At present, due to the large difference in the sensitivity of raw materials between reagents, most coagulation projects do not have uniform calibrations, and the end point interpretation of different instruments is different.

All of these have caused the coagulation method to determine the sample at the same time by optical method and magnetic bead method, and the results between different methods are quite different, the correlation is poor, and it is impossible to compare directly.

Requirement 3:

“The emergency laboratory shall formulate specimen collection requirements in accordance with the testing procedures and take a rapid and effective manner to process the emergency examination specimens to ensure the TAT of the emergency examination..."

Emergency patients often have acute onset, severe symptoms, different degrees of disorder in the body system, coupled with the complex sample collection environment, resulting in a high proportion of abnormal samples in emergency samples, and emergency testing needs to be accurately reported in a short period of time, so it is crucial to have strong anti-interference ability, accurate and rapid detection methods.

Requirement 4:

“Emergency inspectors should attach great importance to factors that affect the quality of testing..."

At present, the degree of intelligence of inspection equipment is high, and the automatic analysis of the instrument effectively improves the quality of inspection.
